Retiree Dental

Retiree dental coverage, offered through MetLife, is designed to promote good dental health for you and your eligible dependents.

Eligibility

If you are enrolled in retiree medical coverage, you’re automatically enrolled in retiree dental coverage (Traditional option). If you have dependents enrolled in retiree medical coverage, they also receive retiree dental coverage. Coverage and Costs

Coverage

The Traditional option is available in all home ZIP codes, and there is no provider network. You may visit any dentist and receive benefits. This option pays 100 percent of reasonable and customary (R&C) charges for most covered diagnostic and preventive services. Other covered expenses are paid based on a geographic schedule.

If you need dental work costing over $200, you should determine, before treatment begins, what is covered and how much your dental plan will pay. Please see “Predetermination of Benefits” in the Dental Expense Plan SPD, or contact the carrier.

Note: You will receive a member ID card from MetLife. The card will contain your MetLife ID number, which you can use for claims and customer service. If you need additional cards for yourself or your dependents, you may download and/or print them from the MetLife website at metlife.com/mybenefits. If you haven’t already, you can register at metlife.com/mybenefits. Simply enter “US-Nokia” in the “Employer or Association” field and then follow the on-screen prompts.
